Your own website (not required for IBCLC’s)
Certified or Licensed in your speciality
NPI Number (if you need one to be created, there will be a $50 fee that must be paid prior to it being created)
CAQH ID (if you need one to be created, there will be a $50 fee that must be paid prior to it being created)
Infant and Adult CPR Certified (up to date and must be from either American Red Cross or American Heart Association)
Liability Insurance (up to date)
Drivers License
Headshot (if applying as an individual)
Personal information will be requested as part of onboarding process
Commitment to completing ALL required forms within 2 weeks of acceptance
Commitment to enrolling in required system: Square payroll

15% billing fee for all paid claims that are billed for all specialities except IBCLC’s
IBCLC’s only: 25% billing fee for all paid claims that are billed
